Default Form Generator Website

I am looking to jazz up some of the look and feel of my website and wonder does anyone know a good html/css form generator so i can build up my form requirements then hone the style and then download the source code for use on my site?

Thanks in advance. Have come across a few but none that create forms with a simple/slick modern look and feel.

There is woofoo, I've tried it before when it first launched. I haven't used it since, but mostly because I wanted to do my own forms and learn. It was, and probably still is, a good service.

Thanks Tony. What i am really after is a form generator that will give me nice looking simple sleek form styles so i can use the css as opposed to the html form code as such.

w3school site provides many solution for CSS or HTML.It can be helpful for you.
