That depends on the situation, different sites have different issues that allow something like that to happen. Hacking is extremely specific, when surveying a site to see if vulnerability issues...

Does it give a more specific reason for failure? If I had to guess I'd say its a permissions error, make sure the user that runs PHP had access to both hard drives.

I know this sounds odd but if you are on a shared server they might cause that. Shared host sometimes restrict weird things. I cant think of a reason why none of those variables would be working.
