Default Help on PHP MYSQL Forms Input

Hi all,
I just learn PHP couple days ago, due to some 'trouble' in the company
and my boss asking me to create an employee database
Help on PHP MYSQL Forms Input

With some help of dreamweaver, i finally manage to create a form for employee database
but.....
Sadly....some of the field cannot be recorded by the mysql...

Can anybody analyze the problem?...

Here i upload the source code and the mysql export/dump
I'm sorry i have to use the RAPIDSHARE
because i don't where else to upload....

It would be alot easier if you would post the code in your post instead.

Use PHP tags [php] and post your code
